Consent to Electronic Records and Signatures

This Consent to Electronic Records and Signatures (“Consent”) applies to all agreements, disclosures, notices, contracts, and other records (“Records”) that Formable Inc. (“Company,” “we,” “us,” or “our”) may provide to you electronically through our electronic signing platform (the “Platform”).

This Consent is provided in compliance with the Uniform Electronic Transactions Act (UETA) and the Electronic Signatures in Global and National Commerce Act (15 U.S.C. §7001 et seq.) (“E-SIGN Act”).

1. Your Right to Receive Paper Copies

You have the right to receive any Record in paper (non-electronic) form. You may request a paper copy by contacting:

We do not currently charge a fee for providing paper copies. If that changes, we will disclose any applicable fee before charging you.

2. Right to Withdraw Consent

You may withdraw your consent to receive Records electronically at any time by contacting:

Withdrawal will become effective within a reasonable period after receipt. Withdrawal does not affect the legal validity of Records previously provided or signed electronically.

If you withdraw consent:

  • You may no longer be able to use certain electronic features of the Platform;

  • We may provide future Records in paper form;

  • Your access to electronic signing services may be limited or terminated.

We do not charge a fee to withdraw consent.

3. Scope of Consent

Your consent applies to:

  • The specific transaction giving rise to this Consent; and

  • All Records provided during the course of your relationship with us through the Platform.

4. Updating Your Electronic Contact Information

You are responsible for keeping your email address and other electronic contact information current. To update your contact information, email:

Failure to update your contact information may result in failed or delayed delivery of electronic Records.

5. Hardware and Software Requirements

  • Internet access;

  • A current web browser supporting 128-bit encryption (e.g., Chrome, Safari, Firefox, Edge);

  • A valid email address;

  • Software capable of viewing PDF files;

  • The ability to download, store, or print Records.

6. Demonstration of Ability to Access Electronic Records

By selecting “I Agree,” “Continue,” or similar acknowledgment, you:

  • Confirm you have accessed this Consent electronically;

  • Demonstrate your ability to access Records in electronic form;

  • Confirm your ability to download, retain, and print Records.

7. Changes to Hardware or Software Requirements

If our hardware or software requirements change in a way that creates a material risk that you will not be able to access or retain electronic Records, we will provide you with updated requirements and notify you of your right to withdraw consent without any undisclosed fees or conditions. We will obtain renewed consent as required by law.

8. Voluntary Consent and Electronic Signatures

You are not required to consent to electronic Records or signatures. However, if you do not consent, you may not be able to use the Platform.

You agree that your electronic signature is the legal equivalent of your handwritten signature and that you intend to be legally bound by Records signed electronically.
